Ptbttle, welcome to the forum. At one time I was a paralegal, and I would never attempt to draw up my own legal documents. All it would take is one misplaced or misspelled word to ruin what was intended. Plus each State has their own laws in regard to a Power of Attorney, and those laws are always being updated.

I suggest you hire a lawyer to do POAs for you. IMO, a lawyer written one will stand up better than than oneboff the internet.

Search them for your home state (or the home state of the person who needs to assign a PoA), or go to Legalzoom.com, Rocketlawyer.com etc. They may even have them on your state's website.

What is your question?
You can get forms from the Attorney.
Do know that the person that is "giving" you POA must be competent to legally sign forms appointing someone POA.
