We need much more information from you in order to help:

What kind of assistance do you mean? Caregiving? Financial? Both?

What state/county do you live in?

Where is your brother currently living: in his home alone, in someone else's home, other?

Does he have a medical diagnosis of dementia? Does he have other health problems or physical limitations?

Does he have an assigned PoA?

Without more info I can only suggest that you contact his county's social services and ask for an in-home assessment. He may qualify for some services, like light housekeeping, food prep, hygiene help, but it won't include caregiving.

If he's very progressed in his dementia and is unsafe to be alone then contact APS for his county and report him as a vulnerable adult. They will keep their eye on him until he's "bad enough" to need facility care. Then they will go through the courts to get him a court-appointed legal guardian who will then manage all his affairs and make all future decisions for him.
