According to your profile your mom was home alone when you went in the hospital in February for pneumonia and survived, so why can't you leave her when you take a little vacation? You yourself say that she has improved. You also say that you have 2 very good friends. Can they not keep an eye on her for you while you get a way?
Or you can look into seeing if any of the nursing homes in your area offer respite care, as some do.
According to your profile you actually sound worse off than your mom. Who's going to look after you and your mom when you no longer can?
It sounds like it's past time to be looking at placing your mom in an assisted living facility, where she will be around folks her own age and get to socialize and be more active.
I can only guess that it would help your health issues as well not having to worry/care for her.
You have to now do what is not only best for your mom, but for you as well.
Please take care of yourself.

Mom seems to be able to manage a bit. She might be a great candidate for a week or two at a nice Assisted Living facility.
She could be as active as she wanted to be. She would get the help that she needed when she needed it.
Would mom be willing to give you the break you need by spending a bit of time in AL?
Many places will do a week or longer Respite stay with the hope that at some point it might be considered as a good option.

Pennsylvania has a Caregiver Support Program that assigns a case manager to figure out a plan of action that includes reimbursement for respite care. Contact the Beaver Falls Area Agency on Aging at 724-847-2262.

Hopefully they can help you with planning for your own care too.

I hope you get to enjoy a change of scenery very soon.
